Flight Report Form
There are two ways you can submit a Flight Report:
- Use the form below............or,
- E-Mail the flight information to
HC David
Howells If you choose to file your flight reports via E-Mail, please
make sure that your e-mail contains ALL the information that
is requested in the form below. This method must be
used by AOL users
Directions on filling out the form:
- Select your name and pilot ID.
- Enter your Email address.
- Complete form - for non schedule flights please enter CHARTER in
FLT No. box
- - for departure and destination please use airport name and ICAO
designation, e.g. Atlanta(KATL)
- Remember to use ZULU time.
- Press the Send button at the bottom of the page.
